Has she been tested for ringworm? Also, what food are you feeding her? Does it have soy, wheat, or other common allergens? My boy has a soy allergy that looked like this.

Skin conditions are so difficult to diagnose, as there could be either internal or external causes. I would go back to the vet if I was you. The treatment you have been given is the usual first line attack, they may suggest trying something different next.

    Could it be stress?

I would try some coconut oil on it and switch to a species-appropriate diet :)

Stress in dogs is something we often don't take into account. I only realised to what extent when Georgina had to have her eye ops last year. Her coat became thin and she had some bald patches. I put her on a course of SF50 (can't remember whether I bought it from Viovets or Petplanet) and Evening Primrose Oil. Within three months her coat was back to normal.
